Upload CSV File for Batch Processing

Upload a CSV file containing multiple patients' diagnosis codes for analysis.

CSV Format Requirements:
  • Required columns: patient_id, sequence, icd_code, poa_status
  • Optional column: dx_pr (use "DX" for diagnosis codes, "PR" for procedure codes)
  • Each row represents one diagnosis or procedure code for a patient
  • Sequence numbers must include 1 for primary diagnosis
  • POA status must be one of: Y, N, E, W, U (can be blank for procedure rows)
  • ICD-10 diagnosis codes and ICD-10-PCS procedure codes can be entered with or without decimals
Procedure Codes (Optional):
  • Include an optional dx_pr column to distinguish diagnosis (DX) from procedure (PR) codes
  • Procedure codes are analyzed for comorbidity risk variables only (no POA, MCC/CC, or HCC analysis)
  • Procedures process FIRST to establish risk variable precedence
  • If dx_pr column is omitted, all codes are treated as diagnoses
File Size Limits:
  • Maximum file size: 1MB
  • Maximum rows: 20,000
  • If your file exceeds these limits, consider splitting it into smaller batches
HIPAA Compliance - No PHI:
  • Patient IDs must be de-identified (no names, SSNs, MRNs, or other PHI)
  • Use anonymous patient numbers or study IDs only
  • Do not include patient names, dates of birth, addresses, or any identifiable information
  • This tool processes diagnosis codes only - no PHI is stored or transmitted

Note: When using the dx_pr column:

  • Use "DX" for diagnosis codes and "PR" for procedure codes
  • POA status can be left blank for procedure rows (procedures don't require POA)
  • Procedure codes are analyzed only for comorbidity risk variables
